Job Description
Responsible for providing individual, family, group therapy and case management services to youth and families.
Detailed responsibilities:
• Conduct intakes and assessments; provide individual, family and/or group therapy, provide case management per client need; utilizes evidenced based curriculum techniques as required by grant.
• Responsible for completing all daily, weekly and monthly progress notes, treatment plans, treatment plan reviews, progress updates and discharge plans for each youth and family on case load.
• Responsible for all pre/post data collection and grant required outcomes per youth and families on case load.
• Responsible for communicating with referral sources on youth/families progress and outcomes.
• Responsible for transportation coordination for special activities and events.

• Masters degree in Social Work, Marriage and Family Therapy, Psychology, Mental Health Counseling, or related therapy field.

Required Work Experience: No prior work experience required.
