Job Description:
Our client is on the lookout for a passionate Care Manager in Social Work for their Sioux City, IA location. The role focuses on assisting patients with chronic illnesses, particularly within their homes. The ideal candidate possesses an active LCSW or LMSW license in IA, has a minimum of 2+ years in care management, and is familiar with medical environments like home health, dialysis, and hospice. The role demands individuals who can work independently, handle telehealth elements, and are keen to be a part of a dynamic startup setting.
Primary Duties and Responsibilities:
- Conduct in-home care management visits to evaluate social and behavioral status.
- Collaborate closely with the Care Team to ensure consistent progress on all care management goals.
- Assess social determinants of health needs and formulate a strategy for addressing them.
- Conduct behavioral, environmental, and social support assessments and surveys as required.
- Offer individual, family, and group education regarding living with chronic illnesses.
- Engage family and support groups in the education and care of patients.
- Evaluate patients and redirect them to behavioral health specialists if diagnosis and treatment are needed.
- Assist patients in comprehending, accepting, and adhering to medical and lifestyle recommendations.
- Serve as the primary contact for patient inquiries about social and behavioral aspects.
- Facilitate proactive care decisions discussions, especially concerning transplantation, home modalities, and AV fistula placement.
- Initiate patient connections through enrollment and onboarding processes.
- Document patient updates and progress in the care management platform.
- Identify and build relationships with local Community-Based Organizations.
- Introduce patients to suitable resources, championing their needs.
- Serve as the subject matter expert on social determinants for the Care Team.
Required & Preferred Skills, Qualifications, and Experience:
- Master's Degree in Social Work, behavioral sciences, or related field.
- Minimum of 2 years of experience in care management, especially with chronic illness within medical environments like home health, dialysis, or hospice.
- Proficient in Microsoft Office and adept with mobile phone and web-based applications.
- Exceptional verbal communication skills, both in person and over the phone.
- Ability to express empathy and rapidly establish relationships with patients and local CBOs.
Required Licenses, Training, and Certifications:
- Licensed Clinical Social Worker (LCSW) in Iowa (IA) or Licensed Master Social Worker (LMSW) in Iowa (IA)
- Valid driver's license and reliable transportation.
