This position assists in providing for the health and wellness of members of our religious community. The Life Care Coordinator works collaboratively with individual sisters, identified companions, and the Life Center/Facility Administrators, as appropriate, to assess, plan, facilitate, and advocate for options and services to meet needs of the Sister. The Life Care Coordinators assist the Sister in achieving wellness and autonomy through advocacy, communication, education, identification of service resources, and service facilitation.
This position serves Sisters who generally reside in Cedar Rapids, Iowa and surrounding areas. Frequent travel across this area is expected. Infrequently, overnight travel may be required.
We are seeking a compassionate and experienced RN or Social Worker to join our team. The ideal candidate is a Registered Nurse, Licensed Social Worker or Psychologist who has a minimum of three years-experience in working with aging individuals to ensure that health & wellness needs are adequately and consistently met, and you have a desire to give back to these remarkable women, this position would be wonderful opportunity-.

QUALIFICATIONS Education, Competencies and Experience
To perform the job successfully, an individual should have the following education, competencies, and experience:
Required:
· Minimum of a Bachelor’s degree in Nursing, or Master’s Degree in Social Work, Psychology, or Gerontology or other related profession.
· Professional licensed in the State of Cedar Rapids in clinical area of nursing, social work, or psychology.
· Minimum of 3 years’ experience with older adults.
· Ability and willingness to work flexible hours
· Ability and willingness to travel by both plane and car when needed.
· Excellent computer skills
· Ability to relate well with the elderly.
· Demonstrates personal integrity and a commitment to ethical principles.
· Demonstrates flexibility and a sense of humor.
· Demonstrates the ability to make a commitment to the mission and values of the Sisters of Mercy.
· Demonstrates the ability to work well with others, including those with diverse backgrounds.
· Demonstrates experiences and skills; and to facilitate effective collaboration through the use of problem-solving skills; conflict management, and other appropriate strategies.
· Possesses the ability to make independent decisions when circumstances warrant such action.
· Demonstrates the ability to deal tactfully with personnel, residents, family members, visitors, Sisters and the general public.
· Must possess the ability to plan, organize, develop, implement, and interpret the programs, resident care goals, objectives, policies and procedures, etc., that are necessary for providing quality nursing care.
· Must possess patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents/families.
· Demonstrates the ability to relate information concerning a resident’s condition as defined by HIPAA regulations.
· Demonstrates respect for co-workers and responds to needs of residents by complying with facility policies and procedures.
· Freedom from illegal use of drugs and freedom from use and effects of use of drugs and alcohol in the workplace.
· Persons who have been found guilty by a court of law of abusing, neglecting, or mistreating individuals in a healthcare setting are ineligible for employment in this position.
· Must have an MVR record with no moving violations. A valid CA Drivers license is required.
· Successfully complete pre-employment requirements, including a physical, drug test, tb test and criminal background check and employment background check.
Preferred:
· Understanding and appreciation of the traditions and culture associated with the Sisters of Mercy.
· Experience with health-related emergencies, health prevention techniques, transportation emergencies.
· Experience with various resources and associated service costs.
· Experience with various service agencies for care and assistance needs in a home environment.
· Experience in navigating easily through health systems and agency systems.
· Familiarity with religious organizations.

Physical Requirements
· Carrying, Writing, Bending, Stooping, Grasping, Standing, Twisting, Sitting, Lifting and Walking.
· Able to push, pull, lift or carry objects up to fifty pounds.
· Perform motor skills such as operating computer keyboard.
· Frequently walk, stand and sit for prolonged periods of time.
· Ability to operate personal computers and standard office equipment.
· Ability to read and interpret documents such as safety rules, operating and maintenance instructions, procedure manuals and policies.
· Ability write and document nursing care and respond to correspondence.
· Sufficient mobility and strength to move freely through the building, to assure Sister safety at all times, and to assist, transfer, or otherwise move residents of facility out of danger in case of emergency.
Visual/Audible Acuity Requirements

Full time - 40hrs a week
$50,000 - $60,000 yearly salaried rate
Schedule:
Day Shift
Must be flexible with the schedule, Day shift, Evening shift, Holidays, Night shift, Weekend availability.
