Responsibilities:
- Conduct comprehensive assessments of patients’ health needs and develop individualized care plans.
- Coordinate and communicate effectively with interdisciplinary healthcare teams, including nurses, therapists, and social workers.
- Ensure compliance with regulatory standards and company policies to maintain the highest quality of care.
- Collaborate with patients, families, and caregivers to address concerns and provide education on care plans.
- Monitor and evaluate patient progress, adjusting care plans as needed in consultation with healthcare professionals.
- Maintain accurate and up-to-date patient records in accordance with privacy regulations.
- Facilitate communication between healthcare providers and community resources to enhance patient support.
- Participate in regular team meetings and training sessions to stay informed about industry trends and best practices.
Qualifications:
- Bachelor’s degree in Nursing, Social Work, or a related field.
- Previous experience in home healthcare coordination or a similar role is preferred.
- Strong understanding of healthcare regulations, compliance, and quality improvement.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Proficient computer skills, including knowledge of electronic health record (EHR) systems.
