Position Summary:
The Care Coordinator provides coordination, education, and outreach services to patients and their families/guardians. In addition, Care Coordinator II supports care coordination/case management services by removing barriers to the process if they fall within the realm of social work and enlists community resources. Primary responsibilities include assessing social service needs and identifying and addressing social barriers to achieving optimal health care status, such as lack or inaccessibility of housing, transportation, utilities, or food. Additional responsibilities include assisting providers in accessing medically necessary specialty care services and ensuring members access appropriate preventive services.
Key Results Areas (KRAs):
•Completes health risk and psychosocial assessments
•Identifies care needs and goals based upon:
o Assessment findings
o Analysis of claims data
o Provider input
o Patient and parent/caregiver expressed goals
•Establish a Care Plan in conjunction with patients and parents/caregivers, and key providers. Plan shall identify:
•Identify community-based organizations that can address patient needs
•Educational needs of patient and/or caregivers to enhance self-management and appropriate utilization of providers to address psychological, developmental, and/or medical conditions
•Organize and facilitate Interdisciplinary Care Team (IDCT) meetings to ensure communication and coordination between patients, parents/caregivers, and key healthcare and support service providers.
•Monitor clinical and functional status of patients through regular face-to-face contact and convenes IDCT meetings on an ad hoc basis to address gaps in care or barriers to successful implementation of the Care Plan.
•Maintain and ensure the confidentiality of all Personal Health Information (PHI) collected and disseminated, in accordance with HIPAA requirements
•Provides patients and parents/caregivers with education required for care in home/community
•Directs the Community Health Workers/Patient Engagement Reps, as appropriate, concerning:
o Scheduling appointments
o Linking patient and family/guardian to community-based organizations and services
o Follow up with/support for patient and family/guardian
o Communication with/support for providers
•Participates in relevant continuing education offerings to enhance the knowledge base needed to oversee care for specialty populations.

Requirements
Core Competencies
- Excellent interpersonal and analytic skills to accomplish assessment calls and coordination
- Knowledgeable about and compliant with laws and procedures regarding patient confidentiality, including HIPAA, Americans with Disabilities Act, and other laws protecting patient rights
- Demonstrated ability to maintain strict confidentiality of client, company, and personnel information
- Proven written and verbal communication skills
- Committed to supporting UCH's standard to medical excellence
Education & Experience:
- At 3 years of experience as a Certified Medical Assistant or higher licensure
- Associate's degree in Nursing or Social Work preferred
- A minimum of 1 year of Case Management experience is strongly preferred
