SUMMARY:
The Care Coordinator is a 2 year grant funded position working with clients who have a history of substance use or other addictions in addressing their supplemental needs that will promote the client’s recovery goals and overall wellbeing. The Care Coordinator duties include but are not limited to coordinating care with primary care providers, mental health providers and/or legal entities, providing information, referrals and/or linkages to available community resources that can address housing, nutritional and financial needs, etc.
MUST BE ABLE TO COMMUTE FROM OUR NF OFFICE TO OUR LOCKPORT OFFICE ON A REGULAR BASIS, with set schedule at each location.
ESSENTIAL DUTIES & RESPONSIBILITIES:
Includes the following. Other duties may be assigned.
· Maintains client chart in accordance with existing Federal, State and clinical regulations.
· Meets with clients on an individual basis to assess for needs and document in coordination of care notes including appropriate signatures and dates.
· Participates in case conference, clinical supervision, and staff meetings as needed.
· Promotes a harm reduction model from alcohol and drugs.
· Interviews clients, reviews records, and confers with other professionals to evaluate condition of client.
· Completes updates to primary care providers, mental health providers, legal entities, and other providers as needed.
· Completes referrals and applications for housing, higher level of care, financial assistance, and other services as needed.
· Engages in case conference meetings with outside agencies as scheduled for coordination of care.
· Promotes Northpointe Council Inc with other community agencies and providers.
· Provides outreach and prevention counseling to the general public and community groups.
Education and/or Experience:
Minimum: Bachelor’s degree in a Human Services or similar related field. Preferred with course work in substance use AND/OR one year practical experience – paid or internship.
