You will be the responsible to guide our clients to a better-quality life with the help of the caregivers, for this you will oversee assessing clients and gathering relevant information about their specific case, and work as a team with their families, community members and the Right at Home team.
A key to succeeding in this social worker/Care Coordinator role is the ability to empathize with the clients and caregivers needs, specially when they are upset. You should be aware of others reactions and understand their situation from their point of view.

Responsibilities:
- Develops and maintains the client care plan, creating extraordinary client experiences and communicating with clients, families, care partners and referral sources.
- Plays an integral role in interfacing with clients and families to ensure safe, appropriate, and meaningful care is being provided.
- Orients new caregivers and serves as their mentor while supporting their care delivery.
- Maintains a calm, positive demeanor always and can effectively multitask in a high functioning office environment.
- Answers phone calls from clients, caregivers, and new prospective clients with the goal of converting to actual clients.
- The ideal candidate has prior experience working in social work and/or in the home care industry or medical office setting.
- High school graduate or equivalent with two years of business experience.
- Have a valid drivers license and insured automobile.
- Knowledge and ability to develop treatment plan, understanding the needs of the client.
- Comfortable using a computer for various tasks, including Word, Excel, Outlook, and Power Point.
- Be available as required for on-call duty outside of normal office hours.
