The Care Coach helps to support the Care Coordination Contact Center and Advanced Illness team in the navigation of patients and their families in the post-acute space. This role will be responsible for taking non clinical inbound calls and escalation to a nurse, as well as to support referral coordination for patients who need CenterWell Home Health Services. Essential Functions:
- Act as a CenterWell Home Health representative in supporting patients who have discharged from service or for those who may need post-acute services. Able to assist the patient and family in accessing pre-determined healthcare resources.
- Assist with patient appointment scheduling when needed
- Assists in the discharge process by sending referrals to alternate levels of care when applicable and, following up with referral status through discharge.
- Participates in special projects and performs other duties as assigned.
- Adheres to and participates in Company’s mandatory training which includes but is not limited to HIPAA privacy program/practices, Business Ethics and Compliance programs/practices, and Company policies and procedures.
- Reviews and adheres to all Company policies and procedures.

Required Experience/Skills:
- Bachelor’s degree preferred
- Minimum of 2 year working in a healthcare related field or healthcare call center
- Ability to learn and master information related to locations and services of client.
- Outstanding computer skills.
- Ability to communicate effectively with empathy over the phone and while interacting with others; excellent interpersonal skills.

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$47,700 – $65,600 per year
