SUMMARY
Family Service League is seeking a full-time Case Manager for our Mental Health Clinic in Huntington. The Case Manager will provide direct service to children, adolescents, and adults with a full range of DSM diagnoses within the clinic. The Case Manager will report to the Program Director and work closely with the medical staff to complete documentation, maintain tracking rosters, and assist with applications.

RESPONSIBILITIES
- The Case Manager will work with the clinic supervisory staff to coordinate behavioral health and primary care for the clinic clients (children, adolescents, and adults).
- Work with primary care staff to schedule clients for services.
- Work with clinic staff to link clients to community resources and other clinical providers.
- Coordinate with other Agency departments, including Health Homes.
- The Case Manager will provide outreach to clients to facilitate their engagement in services and offer additional support so they can engage in treatment and live successfully in the community.
- Provide coaching and recovery support, especially on issues of self-advocacy, linkage to aftercare, wellness self-management, medication adherence, smoking cessation, entitlements, and daily living activities.
- Maintain documents, records, statistics, and other related reports in an organized, timely and accurate manner as per policy and procedure.
- The Case Manager will coordinate care planning with other providers of services/ resources to ensure goal directed, collaborative care, including care transitions.
- Act as a resource to all team members on psychosocial and/or substance abuse issues and resources.
- Provide telephonic as well as face to face outreach, engagement and service planning during clients stay at the center and immediately following discharge.
- Complete training and maintain proficiency with de-escalation and crisis intervention techniques.
- Establish and maintain positive working relationships with others, both internally and externally.
- Foster teamwork, work cooperatively and effectively with others to set goals, resolve problems, and make decisions.
- All other duties as assigned.
QUALIFICATIONS
High School Diploma or equivalent required.
Minimum of 1 year of experience working in a behavioral health environment required.
Knowledge of Medicaid, Social Security, and other entitlements preferred.
Proficient computer skills, including Microsoft Office and Electronic Health Records required.
Strong written and verbal communication skills required.
Valid and clean NYS Driver’s License required.
Ability and willingness to travel in own vehicle to different locations and/or work remotely with other clinic staff required.
Bilingual in Spanish required.
