Description

Position Summary:

The Case Managers are responsible for the case management of the youth in care, and to ensure all program services are provided in accordance with ORR Policy and Procedures, as well as being responsible for the maintenance of the UC Portal. The expectancy is to meet with youth at the minimum twice per month, and more frequently if necessary. When necessary, the case management staff are required to also work collaboratively with agencies that conduct home studies and provide post release services.

Essential Functions and Responsibilities:

  1. Responsible for working with the child and the child's family/sponsor. This includes locating family members, assessing potential sponsors to unify the child with their families/sponsor.

  2. Is sensitive to and supportive of the children’s cultural and ethnic heritage and values.

  3. Assesses the needs of each unaccompanied child in care, while developing an Individual Service Plan, and facilitates a safe timely release/discharge, in collaboration with the Clinician.

  4. Plans with the Clinician for conferences, phone calls, and family visits which are to be coordinated with child care according to team plans.

  5. Responsible for maintaining case files on his/her caseload and document the provision of services in each UC’s case file while maintaining ORR/UAC database compliance.

  6. Responsible for completing initial intake, assessments, case reviews, and weekly case notes to meet programs and ORR deadlines.

  7. Responsible for the facilitation of the preliminary Casey Life Skills Assessment.

  8. Responsible for being the main point of contact between the youth and the local school district.

  9. Collaborates with the Clinician for evaluations and team recommendations/ safety planning.

  10. Participates in multidisciplinary team conferences and meetings.

  11. Coordinates the discharge or transfer of each child including post release services.

  12. Reports and discusses cases with third party reviewers, clinical staff, and the Federal Field Specialist.

  13. Responsible for continued professional development through attendance at in service seminars, participation in conferences and workshops and/or continued study at higher education institutions.

  14. Attends all agency mandatory training as scheduled, and must complete 40 hours annually of ORR (Office of Refugee Resettlement) training.

  15. Fulfills Mandated Reporter responsibilities according to NYS Justice Center and OCFS regulations.

  16. Is familiar with and adheres to all agency policies and procedures as outlined in the Child Care Manual, Employee Handbook.

  17. After hours on call duties, as assigned.

  18. Completes any additional duties assigned by the supervisor.

 

Schedules:

  • Monday 2pm to 10pm, Tuesday through Friday 9am to 5pm with rotating on call responsibilities.

  • Monday through Wednesday & Friday 9am-5pm, Thursday 2pm-10pm with rotating on call responsibilities.

  • Monday through Friday 9am-5pm with rotating on call responsibilities.

Qualifications

MINIMUM:

  • Bachelor’s degree in behavioral science, human services or business administration management or other related field.

  • Related experience in a child welfare agency

  • New York State Driver’s License in good standing as determined by the Agency

PREFERRED:

  • Bilingual in English/Spanish

  • Master’s Degree in Psychology, Human Services, or other related fields.

  • Related Child Welfare and/or case management experience.
