Elderly Housing Management, Inc is seeking three Full Time Resident Service Coordinators to work in Connecticut, in various regions.
Full Time: 37.5 hours/week
The Resident Service Coordinator is responsible for coordinating the supportive service needs of residents, acquainting them with supportive services provided by public agencies or private entities within the community. He/She assists residents to utilize the services that will enable them to remain safely independent, or helping with the transition to another living situation, when Social Services brought into the home are not sufficient, while encouraging, supporting and abiding by, all EMH safety regulations.
Position Responsibilities
Fiscal: Stabilizes community population reducing turn-over and vacancy loss. Render available valuable services to the resident population.
Physical: Monitors property well being during routine resident interaction. Prevents property damage by monitoring resident needs and acting appropriately (i.e. pet placement).Enhances lease compliance with housekeeping standards by making resident aware of available necessary services (i.e. home health workers).
Regulatory: Attends EHM and external training sessions and other opportunities offered by regulatory agencies and trade associations to better equip himself/herself for the job, including the completion of all required training and certification programs, i.e.: the aging process, elder services, disability services, eligibility and procedures for applicable Federal and State entitlement programs, legal liability issues relating to providing service coordination, drug and alcohol use and abuse, mental health issues. Assists residents in meeting compliance with lease and occupancy standards by acting as a liaison to community services and staff
Resident Services Coordination: Refers residents and/or their families and/or Personal Contact to service providers in the general community, including resources for case management, personal assistance, homemakers, meals-on-wheels, transportation, counseling, visiting nurse, preventive health screening/wellness, and legal advocacy, etc. Monitors the on-going provision of community services and advises case management and provider agency of individual's progress. Monitors the provision of supportive services where appropriate. Collaborates with the resident, resident’s family and services to ensure the best quality of life within the community as long as it is supported. Calls on all new residents within the first two weeks of the residents move-in or sooner if indicated or requested and distributes a “Welcome Packet” describing RSC services and community services. Calls on or is in touch with residents during stays in hospitals and nursing homes. Utilizes web based RSC software to document services provided and any incidents or changes in the resident, keeps copies of any paperwork completed on behalf of the resident and updates record as necessary. Is responsible for maintenance and yearly up-date of file of life form after Administrator, or OS, does initial move-in form.
Communication: Establishes relationships with all agencies and service providers in the community, evaluating services to assure individualized, flexible, and creative services for residents in need. Attends the facility Board meetings as required, presenting a written RSC report. Cooperate with the management team to foster a resident focused environment. Prepares and provides monthly report to Director of Resident Service. Serves as a liaison, and facilitates community interaction, between the residents and community services.
Professional Requirements
· Professional degree or equivalent training in fields of Social Services
· Strong communications skills; professional and effective interpersonal skills and comfortable and respectful familiarity with the elderly
· Ability and desire to learn, willingness to take direction and to function as part of a team
· Excellent attention to details and good organization skills
· Ability to deal with multiple priorities and creative problem solving skills
· Computer proficient
Physical Requirements
Moderate physical effort required; frequently seated with freedom of movement. Mobility through the facility and the grounds required. Normal lifting, carrying, reaching, pushing and pulling. Ability to operate a motor vehicle for travel to meetings and training sessions. Visual activity requiring close attention to paperwork and use of computer keyboard.
