```Job Overview:
We are seeking a highly skilled and experienced Behavioral Health Manager to join our team. As a Behavioral Health Manager, you will be responsible for overseeing the delivery of Behavioral Health services and ensuring the highest quality of care for our patients. This is a key leadership role that requires strong clinical expertise, excellent communication skills, and the ability to effectively manage a team.
Position Responsibilities:
- The Behavioral Care Manager supports ECM members with Behavioral Health conditions, with particular attention to ECM members with SMI and/or SUD needs, through brief interventions and behavioral activation
- Will case manage members with Behavioral Health needs, majority Tier 1 and Tier 2
- Responsible for utilizing brief Behavioral Health therapeutic interventions as necessary to improve the Member’s ability to manage their own
- Responsible for brief crisis interventions and warm hand-offs to local crisis resources as needed to
- Provides Member and family education about chronic medical and Behavioral Health conditions to improve health
- Assists with the coordination of medical and Behavioral Health access issues with PCP offices, specialists, and ancillary
- Advocate for members to receive the highest quality care, in a timely manner, within IEHP’s
- Provide effective Care Management for assigned members, including care coordination, linkage to resources, support in transitions of care, facilitating member self-efficacy and self-management, and all other activities associated with high quality, evidenced-based care
- Engage and collaborate with staff outside the BH department, or with those outside BH discipline, to provide integrated care to assigned
- Responsible for assisting Members with care coordination needs, including, but not limited to the following:
- Conduct comprehensive, holistic assessment according to the scope of clinician-both telephonically as well as in
- Aid RN CM in overall review of member Community Health Assessment (CHAs) upon completion by another care team
- Works with Members to identify health/wellness goals and incorporates these goals into the Member’s Shared Care Plan that facilitates communication among Members and
- Participate in inter/transdisciplinary care team meetings to share information, update, and inform care
- Coordinate with internal and external health partners to support Members’ comprehensive care
- Responsible for ongoing engagement with Members, both in-person and on the phone, in a manner that utilizes evidence-based approaches (such as Motivational Interviewing) that promotes collaboration between the Member and his or her medical/behavioral team, as well as to increase the Member’s sense of control over their whole
- Engage in proactive, member-centered utilization and quality review of Behavioral Health and Care Management services by

Position Requirements:
Education & Experience: Master’s degree in social work or counseling. 1-2 years’ experience in social service setting, such as case management or community health. Experience with an EHR/EPM; eClinical Works (ECW) experience preferred. Care Management experience (preferred)
Language: Fluent in English (written and verbal). Bilingual preferred.
License/certification: Valid, unrestricted state LCSW or ACSW. Current and valid Driver’s License and proof of auto insurance.
Skills required: Competent with computers, email, virtual platforms, Excel, and other Microsoft Office based programs. Must have skills and resilience to tolerate a moderate level of change and development around new models of care and care management practices. Excellent verbal and written communication skills, including the ability to convey and exchange information in a clear, effective manner. Ability to identify problems and use logic and related information to develop and implement solutions. Ability to work independently and carry out assignments to completion within the parameters of established policies and procedures. Preferences (not a requirement for the role): Speak Spanish
4+ Years of experience in healthcare or social service Proficiency in EMR, Microsoft programs
Travel Requirements:
Hybrid Work Schedule – 30% of duties will be performed remotely, 70% of duties will involve traveling to conduct in- person member visits.

Pay: $70,000.00 - $80,000.00 per year
