Access BHC, PC is seeking a California Licensed Mental Health Nurse Practitioner (PMHNP) to evaluate patients in an outpatient setting.
Who We Are…
Access BHC provides integrated behavioral health care using the CoCM Collaborative Care Model with physician practices throughout California. We provide therapy for children, adolescents and adults. There is a focus on depression, anxiety, trauma-related disorders and crisis intervention.
We are looking to expand our practice to include medication evaluations and management. We are looking for bilingual (English/Spanish) PMHNP’s licensed in California who are currently enrolled Medicare providers or are eligible to become Medicare providers.
ESSENTIAL FUNCTIONS:
- Conduct initial medication evaluations.
- Develop treatment plans based on evidenced-based standards of care and practice guidelines.
- Conduct medication follow up appointments with clients on ongoing basis to meet treatment needs.
- Ensure all documentation regarding patient care, treatment, and incidents are completed in a clear, concise, and timely manner.
- Develop and maintain effective personal organizational systems for paperwork / information management in order to support timelines and priorities.
- Respect and uphold patient confidentiality.
- Attend monthly video or phone meetings with oversight team.

- Active Medicare Provider with PTAN (preferred)
- Active Medi-Cal Provider (preferred)

Pay: $65.00 - $85.00 per hour
