Behavioral Health Solutions (BHS) is seeking a hard-working, self-driven individual to join our behavioral team as a Behavioral Coordinator. A Behavioral Coordinator is a healthcare professional who is responsible for ensuring that patients receive the best possible care for their mental and behavioral health needs. The role of a behavioral care coordinator involves working with physicians, psychiatrists, psychologists, and other healthcare professionals to develop and implement strategies to improve patient outcomes.
The ideal candidate will have already worked in a Skilled Nursing Facility (SNF) or Behavioral Health Unit prior, providing a variety of services to patients/residents. CNAs, BHTs, Medical Assistants, and similar personnel are encouraged to apply!
This position will train with and report to a remote Clinical Supervisor for oversight on all cases worked.
Responsibilities include:
- Participate in clinical training/case consultation as needed and biweekly clinical oversight with clinical supervisor.
- Provide behavioral health assessment and therapeutic interventions to individuals and/or groups in long-term care settings under direction of a clinical supervisor.
- Document assessments and progress notes for each encounter and submit within 24 hours for review by clinical supervisor.
- Conduct therapeutic program implementation as requested by partner facilities and approved by clinical supervisor.
- Perform non-violent crisis intervention as appropriate.
- Participate in case management functions as requested by a partner facility and/or care management personnel.
