New Alternatives for Children, Inc. (NAC) is an award-winning not-for-profit agency in Midtown Manhattan, with a satellite Bronx office, dedicated to serving children and families with medical complexities, chronic illnesses, and disabilities throughout the five boroughs. To assist in achieving this mission, NAC is currently seeking a Bachelor’s LevelCase Plannerto join its Special Medical Prevention Program.
Our Prevention program delivers a range of clinical, case management, and supportive services to families who are child-welfare involved and caring for children diagnosed with chronic or complex medical conditions. The goal is for families to remain intact while ensuring caregivers receive the appropriate support to meet all of their children’s physical, developmental, and mental health needs.
The ideal candidate will be strength-based, trauma-informed, culturally responsive and committed to family preservation and resilience. This is a city-wide home and community-based program and the candidate must have the ability to work independently and flex their hours on weekdays to meet the needs of youth and caregivers. Our hybrid model supports work-from-home when staff are not in the field. Description of Responsibilities:
· Case Planner provides casework and clinical services to youth and caregivers utilizing individual and family modalities. Coordinates services for a caseload of 8-9 families as recommended by NAC’s multidisciplinary treatment team.
· Conducts home and community-based visits.
· Adheres to agency and ACS regulatory requirements related to casework and collateral contacts, Family Team Conferencing, Workforce Institute training, and timely completion of Family Assessment and Service Plans (FASPS) and progress notes in CONNECTIONS.
· Case Planner makes adequate use of individual, group and peer supervision, clinical consultation, mental health in-services, and social justice discussion forums.
· Case Planner collaborates with psychiatrists, psychologists, pediatricians, nurses, case associates, and resource and education specialists in a dynamic, strength-based interdisciplinary team model.
Required Education and Experience:
- · Bachelor's degree in social work, human services, psychology, or a related field
- · Minimum 1 year Prevention Case Planning experience
Required Skills:
- · Excellent written/verbal communication and organizational skills
- · Strong engagement skills
- · Self-awareness and motivation around team building
- · Proficiency in Spanish is preferred (and includes a salary differential)
