Erosun Inc. is now hiring an Assistant Program Director. The Assistant Program Director provides quality management oversight to participants with intellectual and developmental disabilities and their direct support professionals who work in home like settings. The right person for this position will ensure that our employees are helping the people we support to learn independent living skills, plan activities in the home and community, and assist with the implement of person-centered plans (PCP).
This position requires a professional with Coordinator of Community Service (CCS) experience and must be knowledgeable in operating the LTSS system.
In addition to advocating for the people living in the home, other responsibilities include:
- Researching for new apartments for potential admissions,
- Assist with investigating incident reports,
- Participate in program meetings,
- Assist with provision of timely and accurate documentation related to the individualized plans, and person-centered plans.
- Ensuring that MANDT supports and nursing care are available. (The MANDT System is a comprehensive, integrated approach to preventing, de-escalating, and if necessary, intervening when the behavior of an individual poses a threat of harm to themselves and/or others).
- Reports medical concerns, conditions, and changes in health status in a timely fashion.
- Provides outstanding support and services for people with intellectual and developmental disabilities, enabling them to participate as fully as they wish in all aspects of community life.
- Ensures a safe environment where individuals are free from abuse and neglect.
- Ensure proper documentation and reporting of incidents are investigated and areas of concern corrected.
- Ensure accuracy, completeness and implementation of nursing assessments, nursing care plans, nutritional evaluations, choking risk assessments, MAR’s, and all other related medical documents
- Provides clear and concise documentation and follow-up on all employee concerns and disciplinary actions
- Ensures individualized plans (including MANDT plans and medication plans) are up-to-date and accurately reflects each individual supported. Ensures employees are well trained and are implementing the plan(s), Ensures proper distribution and documentation to all involved parties is completed within the designated timeframe.
- Fill participant vacancies in the residential homes as needed including acting in the needed role, and participates in weekend on-call rotation.
- Assist with completion of the Detail Service Authorization Tool (DSAT) and Cost Details.
Salary: Negotiable.
