Annual Wellness Nurse Practitioner

Core Healthcare Alliance (CHA) is a Management Service Organization committed to improving overall health and wellness to Medicare Advantage members.

Job duties:

• Prepares for patient visits by identifying screening tests to be performed, reviewing past medical records and coding notes
• Reviews and interprets screening test results and educates patients accordingly
• Completes a comprehensive H&P using approved documentation templates
• Identifies all significant acute and chronic conditions, and provides case-specific education and care planning
• Identifies patients at high-risk for complications and collaborates with Care Management team and to coordinate primary and specialty care if needed
• Accurately and thoroughly log all documentation in a timely manner
• Closes identified gaps of care for HEDIS
• Captures chronic conditions
• Occasional travel to other clinic locations in Carson City and Fernley
• Other duties as assigned
