General Description:
Under the supervision of the Director of Pharmacy and the Ambulatory Care Clinical Pharmacy Coordinator, the Ambulatory Care Clinical Pharmacy Specialist delivers patient-centered care, advises, consults, and provides drug information to patients, their care takers and health care professionals in the Primary Care Clinic. The Ambulatory Care Clinical Pharmacy Specialist assists the Ambulatory Care Clinical Pharmacy Coordinator in the implementation and enforcement of standards according to department and medical staff policies and procedures.
ROLES AND RESPONSIBILITIES

  1. Complies with established corporate and departmental policies, procedures, objectives, quality assurance methods, and safety codes. Demonstrates compliance with licensing, regulatory and accrediting agency provisions as required.
  2. Consistently demonstrates appropriate and professional communication behavior toward patients, customers, and coworkers. Cooperates with Management and peers to promote an environment of teamwork and collaboration.
  3. Reads, extracts, and interprets information in patient charts appropriately.
  4. Utilizes disease state and medication assessment per evidence-based care plans, protocols, and clinical judgment. Provides direct patient care related to chronic disease state management and medication therapy changes under a collaborative practice agreement.
  5. Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed. Assists physicians/office staff with any medication questions, insurance issues, hospital follow-up visits, medication reconciliation, refills, referrals, diabetes education visits, etc. Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-disease interactions, drug & food interactions, drug-laboratory test interactions, allergies, appropriateness of therapy, and therapeutic duplications.
  6. Provides education to patients and caregivers regarding medications and disease states. Clarifies any medication questions with any of the patient's physicians/specialists when needed.
  7. Utilizes standard work while in patient visits and during telephone calls.
  8. Documents all clinical activities and interventions accurately and completely.
  9. Screens patients for immunization status and documentation in Health Record with accuracy. Provides immunizations when needed if certified following standard work.
  10. Collaborates with physicians, nurses, or other office health-care professionals regarding drug information and appropriate drug therapy.
  11. Leads and participates in educating Primary Care Clinic providers and staff on topics related to Ambulatory Complex Disease Management. a.)Provides staff development support to nurses, physicians, and other healthcare providers. b) Provides presentations, publications, and other informative activities on medication-related topics to the healthcare community and public. c) Precepts and mentors IPPE and APPE pharmacy students as needed
  12. Attends orientation, education, and training programs. Reviews literature and other materials pertinent to the practice of pharmacy. Attends meetings as required.
  13. Represents Broadlawns Pharmacy Department during community outreach partnerships.
  14. Embrace and support diversity, equity, and inclusion initiatives.
  15. Performs all other duties as assigned or requested.

MINIMUM QUALIFICATIONS
  1. Currently licensed Pharmacist in the State of Iowa.
  2. Must be a graduate of an ACPE- accredited School of Pharmacy with a Bachelor of Science Pharmacy degree or Doctor of Pharmacy.
  3. Basic Life Support certified (BLS).

PREFERRED OUALIFICATIONS
  1. Two years Ambulatory Care pharmacy experience or completion of PGY-1 pharmacy residency.
  2. Certified Immunizer by the State of Iowa.
  3. Current or pending Board of Pharmacy Specialties Certification (BCACP, BCPS, etc.).
  4. Certified Diabetes Care and Education Specialist (CDCES).

Monday - Friday 8:00am - 4:30pm

Further, if offered a position with Broadlawns Medical Center, I also agree to submit to a medical history and physical examination, which includes drug testing, provided by Broadlawns Medical Center and understand that my appointment is conditional on satisfactorily passing the examination prior to being placed on the job. I understand that I will be responsible for providing BMC proof of MMR (Measles, Mumps, rubella) immunity and/or immunization records at my expense prior to beginning my employment.
