We are looking for another healthcare professional with clinical experience to join our team. A minimum of a Bachelor's Degree is required. We have a strong preference for a MSW or LCSW to balance out our team right now, but other successful candidates might be a RN, OT, PT or SLP. Prefer a candidate who is licensed or credentialed in their field and has experience working with people with dementia. As Aging Life Care Managers, we are not providing bedside clinical care, but we use our clinical education and experience to provide exceptional medical advocacy and care coordination.
This position requires seeing clients in Northern Virginia. This is not a remote position.
The ideal candidate will have at least 10 years of experience working with older adults in community based settings and be comfortable managing clinically complex clients. We all have a very strong work ethic and work equally well on our own and as a team and need you to have the same
At Pathways in Aging, we provide Aging Life Care Management services all over Northern Virginia through:
- Comprehensive assessment
- Care coordination
- Crisis intervention
- Consultation
We perform in-person home visits, phone and video encounters. We often work with highly complex clients. We specialize in working with older adults, but sometimes work with younger people. The work we do involves creative problem solving, is challenging and VERY rewarding.

Required:
Fully vaccinated for Covid-19
Willing to transport clients in your vehicle upon occasion
Willing to perform client visits in any part of Northern Virginia
Strong preference for candidates who have the following:
Experience and knowledge base in working with people with dementia and Parkinson's Disease
Experience doing home visits
Knowledge of the area senior serving organizations and businesses
Basic working knowledge of Medicare and Medicaid
Comfortable working autonomously but is also a team player
