The Virginian is actively recruiting for a Social Worker. This full time, leadership role, provides supportive and compassionate social services to Residents and their families at all levels of care. Assists with their social and emotional adjustment throughout residency at the facility, to include admission and discharge process. Supports philosophy of maximizing independence for all Residents to the fullest extent possible. Provides superlative level of service to Residents and families, within regulatory framework.
RESPONSIBILITIES:
- Assists with Residents' admission process, interviews Resident and family to complete initial psychosocial assessment, (new admissions within fourteen days), identifies problem areas and strengths. Develops discharge plans and goals
- Assists Residents & families with Medicare, Medicaid, and other insurance related questions
- Counsels Residents regarding personal or emotional problems
- Documents assessments of Residents on a quarterly basis following initial evaluation and as needed
- Attends individual conferences with Resident, family, and staff, as needed- Maintains periodic contact with family
- Leads/facilitates the individual Interdisciplinary Team meetings for Residents (HCC only)
- Attends Individualized Service Plan meetings for Residents (AL and EAL)
- Represents the Resident and/or family to the staff, interprets social problem areas, plans and goals
- Attends appropriate in-service training programs, maintains 100% compliance with Relias Training
- Maintains Resident privacy and confidentiality at all times
- Completes MDS (Minimum Data Set) according to schedule dictated by MDS Coordinator
- Documents as required in Answers on Demand (MATRIX). Must be familiar with MATRIX assistance, resources and willing to pursue training as required
- Completes MIMR (Mental Illness Mental Retardation) screening form upon HC admission when needed
- Completes and explains Medicare Provider of Non-covered Services Notice (NOMNC)
- Completes Notice of Transfer and Discharge from the HCC as appropriate.
- Supports/ facilitates meetings for Family Councils/Parkinson's/Alzheimer's Support groups only to extent chairmen request help.
- Maintains liaison with community agency representatives
- Facilitates Residents referrals to Partners in Care
- Performs other related duties as assigned by Director of Social Services
- Assist with Resident Council meeting for HCC and/or AL
- Takes the "On Call" role as assigned by Director
- Facilitates transfer of physicians and notifies appropriate personnel
- Attends quarterly AL and/or QA meetings
- Understands QAPI process and participates as assigned
- Orders medical equipment as recommended by physician or other qualified medical personnel
- Completes annual UAI's (and maintains corresponding records) for IL
- Facilitates referrals to LCSW as needed
- Completes ISPs for IL Hospice Residents
- Serves on Safety Committee and attends quarterly meetings
- Facilitates smooth transition into home health services (HH SW only)
- Ensures that HH patients are referred to appropriate services as needed (HH SW only)
- Ensures that HH patients have appropriate medical equipment as ordered by the physician (HH SW only)
- Provides information regarding the HH benefit under Medicare and other insurances to the patient and their families (HH SW only)
QUALIFICATIONS:
- Must have a Bachelor's degree in Social Services, Counseling, related field or a combination including experience
- Excellent communication, documentation and group dynamics skills
- Adopts team-player attitude with all staff
- Must be able to work with frequent interruptions
- Willing to work beyond normal working hours, on weekends and in other positions temporarily, when necessary
- Must be able to provide HH services at client's home (HH SW only)
- Must be have reliable transportation to client's home (HH SW only)
- Maintain privacy and confidentiality of Resident information at all times
- Will attend mandatory in-services conducted within the facility-100% compliancy with database training on a monthly basis
- Must be able to move intermittently throughout the workday
- Must be able to cope with the mental and emotional stress of the position
- Must possess sight/hearing, or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met, to include use of departmental equipment and/or supplies
- Must function independently, have flexibility, personal integrity, and the ability to work effectively with the Residents, personnel and support agencies
- Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times, hostile people within the facility
- May be required to lift, push, pull and move equipment, supplies, etc. up to 25 lbs. per office supplies, getting assistance from porter when needed
- Must be able to read, write, and speak English in an understandable manner
- Must possess the ability to make independent decisions when circumstances warrant such action
- Must possess the ability to deal tactfully with personnel, Residents, family members, visitors, government agencies/personnel and the general public
- Must be able to follow oral and written instructions
- Must have patience, tact, cheerful disposition and enthusiasm, as well as, be willing to handle Residents bases on whatever maturity level they are currently functioning
Compensation for this role will be reviewed only with qualified applicants.
Compensation: $60,000-$80,000 annually with great benefits!
