YOUR IMPACT // Help to empower others by listening to each customer's unique situation and providing options to assist them on their journey. Primary responsibility will be answering incoming customer calls to provide information and assistance, assessment, and long-term care options counseling services to older adults and adults with disabilities in Brown County.
  • Ensure that every story matters through active listening in-person, online, and through the call center
  • Boost confidence and reduce stress by understanding customer and caregiver needs, and providing resources that will assist them
  • Focus on person-centered planning through creation and development of action plans
  • Help to connect eligible customers to programs by administering Long Term Care Functional Screens

  • Experience & Education & Experience: Bachelor's Degree from an accredited college or university in a human service or related field is mandated. In addition, two years' experience in human services or social services, or any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
  • License/Certifications: Valid State of Wisconsin driver's license and a satisfactory motor vehicle record. Functional Screen Certification within 6 months of hire.

JOB SUMMARY:
Provides information and assistance, assessment, and long-term care options counseling services to older adults and adults with disabilities in Brown County. Offer options counseling built on the strengths of individuals, families and the community. Works collaboratively with internal and external services in order to advocate for consumer needs to assure services are delivered.

MAJOR RESPONSIBILITIES:
Consumer Services:
Provides consumer driven prompt, accurate and friendly customer service responses to requests for information about an array of topics and for access to long term care resources.
Collects consumer information and assesses clients/family needs in order to provide information and assistance over the phone, in office or at home that best meets the needs/demands of the consumer's schedule.
Provide follow up as needed to determine outcomes and provide additional assistance in locating resources and arranging services.
Develops and implements action plans, short term, to organize and direct in-home supportive services for individuals with a limited support network according to ADRC policy.
Assists in the identification of individualized, cost-effective, outcome-oriented service options (Options Counseling) that reflect goals that are personally meaningful and attainable by consumers and address immediate as well as longer term needs.
Administer the Long Term Care Functional Screen to determine functional eligibility.
Provide information about the publicly funded long-term care and assist consumers throughout the eligibility determination and enrollment process.
Collect preliminary financial data (including medical and remedial expenses) and refer to Economic Support Unit for financial eligibility determination.
Participate in various transition activities designed to help youth who have a disability transition from school to the adult service system.
Participates in prevention activities to include: evidence based classes, support groups and/or presentations.
Arranges or provides transportation for consumers, if necessary, for service coordination.
Problem solves with internal and external agencies on any concerns with clients as they relate to the independence and needs of the consumers served.
Has a "Duty to Report" during a community emergency/disaster according to the ADRC Emergency Preparedness Plan.

Documentation/Reporting
Completes and maintains appropriate and timely documentation on consumers in the consumer records and databases.
Completes and maintains required agency documentation for 100% Time Reporting purposes.

Outreach and Community Collaboration
Participates on special task forces, internal and community committees, in order to assist coordination of programs and services.
Provides outreach activities in the form of booths and/or public speaking engagements for educational purposes on aging and disability services in the community.
Represents ADRC of Brown County to the community at large through professional interaction, public speaking, media presentations, and participation in community advisory groups as requested.
As an Information and Assistance service, works collaboratively with other Information and Referral programs to provide specialty assistance to agency target populations.

Professional Growth and Development
Attends and participate in staff development programs, including in-service training, staff meetings, and professional seminars including certification requirements in I & A through AIRS, and Functional Screen Certification.
Comply with applicable federal and state laws, administrative rules, established agency procedures and accepted professional standards.

MINIMUM QUALIFICATIONS REQUIRED:

Education and Experience:
Bachelor's Degree from an accredited college or university in a human service or related field is mandated. In addition, two years experience in human services or social services, or any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.

Licenses and Certifications:
Valid State of Wisconsin driver's license and a satisfactory motor vehicle record.
Functional Screen Certification within 6 months of hire.

KNOWLEDGE, SKILLS, ABILITIES
Knowledge of and demonstrated compliance with professional ethics and boundaries standards and codes of conduct.
Knowledge of standards and principles of Information and Assistance including a willingness to become certified as an I & R Specialist through AIRS.
Knowledge of community resources and referral procedures.
Knowledge of assessment and care management techniques.
Knowledge of planning, organizing and problem solving strategies.
Knowledge of personnel management and policies.
Knowledge of public service agencies, laws and regulations governing or affecting aging
and community services.
Knowledge of HIIPPA, privacy, and confidentiality standards of practice.
Ability to relate to older people and people with disabilities and assess their needs.
Ability to deal tactfully with difficult situations.
Ability to communicate effectively, both orally and in writing including the ability to communicate effectively to a variety of types of persons, both individually and in
groups.
Ability to proficiently use a computer and related office equipment; demonstrating familiarity with required software and data base programs.
Ability to establish priorities for service intervention.
Ability to be decisive and to make sound decisions under the pressure of crisis or emergency situations.
Ability to participate in a work setting as a team player.
PHYSICAL DEMANDS:
Lifting 20 pounds maximum with frequent lifting and/or carrying of objects weighing up to 10 pounds.
Extended periods of sitting; intermittent standing, walking and driving.
Using hand(s) for single grasping, fine manipulation, pushing and pulling, and operating controls.
Occasional bending, twisting, squatting, climbing, reaching and grappling.
Communicating orally in a clear manner.
Distinguishing sounds at various frequencies and volumes.
Distinguishing people, objects or injuries at varied distances under a variety of light conditions.Employment Type: Full-time
