The Admissions Coordinatoris responsible for coordinating all aspects of the referral to admission process in compliance with federal and state regulations.
- Communicates directly with all involved (doctor, nurse, facility, family, etc.) regarding the referral of a patient.
- Responsible for communicating with the nurses completing needs assessments to coordinate and schedule timely admissions of patients that desire our care.
- Records any pertinent medical history and assists in obtaining proper medical records that may be relevant to determine hospice eligibility.
- Assists the Admissions Department Director with the management and organization of paperwork related to a referral, needs assessment, and or admission.
- Assists in ordering the E-kit for all new patients admitted per MD order and forwards to the pharmacy.
- Assists in ordering DME equipment as needed.
- Assists in tracking non-admitted patients who are pending so patients may be admitted as soon as appropriate to achieve targeted growth goals.
- Communicates with leadership and sales team daily regarding status of pending referrals.
- Establishes and maintains positive working relationships with current and potential referral and payor sources to bring a greater understanding of the hospice philosophy and support patients and families that desire our care.
- Participates in the on call rotation
- Assist the Admissions Department Director with all other duties as needed.
- Knowledge of and adherence to all policy and procedures.
- Maintains compliance by documenting accurately and timely within the organization’s electronic health record.

- High School Diploma/Equivalency.
- 1 year healthcare experience, hospice preferred.
- Computer experience including knowledge of Microsoft office.
- Primarily frequent in nature with exerting up to fifty pounds of force occasionally, and/or up to twenty pounds of force frequently, and/or up to ten pounds of force constantly to move objects: reaching, lifting, carrying, handling, walking, sitting, standing, bending, squatting, climbing, kneeling, and twisting. Position may require pushing and pulling up to 300 lbs. Frequent to continuous speaking, hearing, near visual acuity, far visual acuity, depth perception, typing and driving.
