Every day you will work closely with patients, families, healthcare providers, and community resources to develop and implement comprehensive care plans that address the patient's needs and goals. You will be responsible for coordinating and managing healthcare services, monitoring patient progress, and facilitating communication between patients and providers. You will be expected to conduct assessments, provide education and support, and ensure that patients receive the appropriate level of care and resources. You will also be responsible for monitoring and reporting on patient outcomes, ensuring compliance with regulatory requirements, and participating in quality improvement initiatives. To be successful in this role, you should have strong communication and interpersonal skills, be able to work independently and as part of a team, and have knowledge of healthcare regulations and policies. Additionally, you should have experience working with electronic medical records and be proficient in Microsoft Office.

Responsibilities
- Works with ACO members providers and caregivers to provide intensive comprehensive case management and increase efficient utilization of services
- identifies chronic or catastrophic cases through the case management process and initiates intensive case management according to program guidelines.
- Effectively manages patients on an outpatient basis to assure the appropriate level-of-care is provided to prevent inpatient re-admissions and ensure that the patients medical environmental and psychosocial needs are met over the continuum of care.
- Acts as an advocate for members and their families linking them to other members of the care team to help them gain knowledge of their disease process and to identify community resources for continued growth toward a maximum level of independence.
Qualifications
Minimum
- Associates degree in Nursing or graduate of accredited school of nursing
- Three years of experience in hospital or managed care case management or utilization review
- Valid compact state or Arizona RN license
- American Heart Association Basic Life Support for the Healthcare Provider
- Intermediate computer proficiency with the ability to compose documents in a professional writing style and basic proficiency spreadsheet applications
- Ability to engage the geriatric population over the phone
- Ability to prioritize plan and handle multiple tasks/demands simultaneously
Preferred:
- Bachelor's degree in Nursing
- Managed Care Organization experience
- Experience with INTERQUAL or Milliman
- Hospice experience
- Electronic Medical Record (EMR) experience; Cerner
- Bilingual English/Spanish
Pay Range
$35.28 - $51.16 /hour
