Responsibilities include, but are not limited to:
Will work with and report directly to the Director of Medical Records to ensure that all requests for medical records are answered in a timely manor while adhering to HIPAA laws and regulations. Will maintain a live and accurate spreadsheet of all Release Of information requests that come in and out of the facility. Will be responsible for obtaining information from outside sources in order to ensure continuity of care is maintained. It is the responsibility of the ROI Clerk to release patient Health Information to persons and agencies in accordance with the rules and regulations governing the Release Of such information; to request and obtain patient Health Information from other facilities/providers with proper authorization and according to policy and procedure. Releases protected Health Information to Attorneys and/or Court systems per valid authorization, subpoena or court order. Processes request for information from Social Security, Disability Determination, Patients and/or others as requested with proper authorization within 10 days from date requested. Will scan and upload outside information into electronic medical record in addition to all consents. This position will also serve as the Notary Public for Medical Records and other Center needsHFC incorporates principles of trauma-informed care throughout the agency, emphasizing cultural sensitivity and a safe and respectful environment for all individuals served and staff. All job descriptions include the responsibility to learn about and implement trauma-informed practices within the scope of work for the position. Reassignment is a condition of employment that may occur at the employer's discretion. Your work location and hours could change based on program needs. Possibly subject to federal vaccination mandate, including provisions for claiming exemptions.
QualificationsEducation, Training, and Experience
Must be high school graduate or GED, one year of medical records experience preferred. Demonstrated job stability. Must have experience working in a fast-paced environment and have experience in automated office systems such as Microsoft Office, Excel, Outlook, etc.
Must have current drivers license and be insurable under Center insurance. Must pass and maintain all Center-mandated trainings.

- Ability to organize and prioritize workflow to meet deadlines.
- Ability to utilize various computer systems, with experience in Word, Excel, and Access.
