The Nursing Department in the Manhattan campus is seeking a full time Registered Nurse Wound Care Specialist to join our team! This position will sit in the Upper West side of Manhattan location.
Job Summary:
We are looking for an experienced and motivated Wound Care Nurse to join our team of clinical professionals. As part of our team, you will help to assess wounds to determine both immediate treatment needs and long-term treatment plans. You will make regular rounds to patients in your care, coordinate with physicians, nurses and other clinical team members as needed regarding case management, and liaise between families and external stakeholders. Before the patient is discharged, you will educate patients and families on care practices and methods to promote wellness and healing. You must be a licensed registered nurse (RN) and have Wound Care Certified (WCC) credentials from the National Alliance of Wound Care and Ostomy to be considered for this position; bilingual candidates are strongly preferred.
Duties and Responsibilities:
- Assess the unique needs of each patient to determine appropriate treatment plans
- Educate patients and families on best practices to promote healing and wellness
- Track treatments and patient progress according to HIPAA guidelines
- Coordinate with clinical team to ensure smooth recovery processes
- Evaluate wounds and injuries for infections and other potential wound complication.
- Liaison with Infection Control Nurse for skin and soft tissue infections and Multi Drug Resistant organisms.
- Round with wound physicians and clinical teams; serve as liaison between wound physicians and clinical staff
- Initiate nursing care procedures in managing acute and traumatic wounds.
- Coordinate with rehab and other disciplines; serve as expert resource re: facility systems such as bed and wheelchair surfaces and other related devices
- Serve as in house Wound Care expert; provide mentoring to Nursing & Medical Staff at point of service.
- Provide regular formal and information in-services to nursing and other members of the IDT
- Demonstrate Wound Care procedures to other care givers.
- Create and manage care plans for patients with complex wounds, ostomies, and/or continence conditions.
- In collaboration with wound physicians, make recommendations to update Wound Care formulary as needed
- Ensure proper documentation is in place
- Track and trend all wounds for QAPI
- Assess and make recommendations for pain management as needed
- Fulfill other nursing duties as required
Requirements and Qualifications:
- BSN degree (strongly preferred)
- Current state licensure as a registered nurse (RN)
- CPR and basic life support (BLS) certifications
- Actual work experience as Wound Care Nurse in long term or acute settings required.
- Prior experience in inpatient and palliative care (preferred)
- Wound Care certification required for external candidates.
Interested Internal RN candidates will be trained and be provided assistance in getting Wound Care Certification.
