RN-FFS Wound Care Nurse

As the Fee-For-Service Wound Care RN you will provide consultation for an in-depth assessment of the patient's wound status and overall condition during the delivery of home care services. In conjunction with the community MD and primary RN coordinator, the Wound Care RN offers therapeutic measures and treatment to promote healing and further wound prevention.

Responsibilities include, but are not limited to:
- Conducts complete assessment of wound and patient's overall condition upon referral.
- Establishes therapeutic measures and treatment to promote healing and further wound prevention in conjunction with community MD and Primary RN Coordinator.
- Reports findings and recommendations from home visits to primary RN coordinator and/or RN Supervisor within one working day of visit on those cases where joint visit is not made with primary RN coordinator.
- Instructs patients, caregivers, and RN coordinator according to the established treatment plan.
- Monitors healing process in conjunction with primary RN coordinator and obtains physician orders as necessary for changing patient needs.
- Submits completed documentation of all home care visits and coordination within seven days.
- Provides curriculum for staff nurses in Wound Care in the home care setting that may include a review of literature, direct classroom teaching, independent modules, and assessment of clinical skills through direct observation.
- Identifies in-service and education programs in Wound Care in the home care environment to ensure ongoing nursing competencies.
Required Qualifications:
- License and current registration to practice as a Registered Professional Nurse in New York State
- Graduate of an accredited school of nursing, BSN preferred
- A minimum of one-year nursing experience in home health agency required
- Certification as a Wound Care Specialist required
- Ostomy certification preferred
- Ability to plan, provide, and case manage services in accordance with the age-appropriate needs and capabilities of the elderly and adult populations
- Access to reliable transportation, and must be able to make home visits in potentially unsafe and unsanitary home environments. Travel on public transportation in potentially unsafe areas of the community
- Lifting up to 50 pounds unassisted and up to 325 pounds utilizing an assertive device in the home environment. In the office, lifting is limited to fields and records not to exceed 10 pounds.
