JOB DESCRIPTION
To be minimally qualified for this position, the candidate must have at least 1-year of Home Health Case Management experience and have BLS and WI RN license (or compact) in hand.
Patients typically have multiple comorbidities and can include (but not limited to): CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, fall, fracture, infections, cellulitis, post-surgical, wound care, ostomy care, labs, IV infusion, etc.
This position will manage 20-30 patients per day.
Any scrub color is acceptable. Traveler must provide their own transportation to/from all patient visits.
Coverage area: Green Bay Market, Brown County, Parts of Outagamie
