Forward Service Corporation (FSC), a passionate, not-for-profit firm that strives to provide exceptional customer service in employment and training related services to local, state and federally funded program participants. Our non-profit firm is seeking one full-time W2 Case Manager to help those we serve achieve their dreams. The home office is located in Jefferson, WI.

The W2 Case Manager provides case management services by assessing applicant’s needs, making referrals and empowering individuals to reach self-sufficiency. The W2 Case Manager inspires hope for our participants and builds their strengths.
PRIMARY DUTIES INCLUDE

• Determine eligibility for W2 and related services
• Monitor reports to ensure compliance
• Provide individual and group services to aid individuals in achieving goals
• Provide on-going case management and maintenance of customer and program records
• Support individuals those applying for SSI or SSDI benefits with application and referral information
• Enter data accurately and timely into appropriate networks
• Facilitate job readiness workshops
• Travel to Dane County 1-2 days per week (required)

EDUCATION/EXPERIENCE

Associates degree and two plus years work-related experience and/or equivalent combination or education and experience. Valid Driver's License and Personal Vehicle Insurance Required.

COMPENSATION & BENEFITS

• FSC offers a very competitive wage for this full-time, non-exempt position with a minimum starting rate of $23.02/hour with additional compensation based on direct experience beyond the minimum requirements.

• Example: Meet only minimum requirements start at $23.02/hour; with 3 additional years of direct experience start at $23.75/hour; 5 additional years of direct experience $24.50/hour, etc.
• Additional compensation of $1/hour award upon completion of training plan
