Veteran's Directed Care Person Centered Counselor / Assessment Support Navigator

Area Agency on Aging Region 9 (AAA9) has a position open for a Veteran's Directed Care Person Centered Counselor and Assessment Support Navigator. This is an in-office position working with Veteran's Directed Care referrals and supporting Veteran's to remain in their homes. Must have high school diploma or equivalent. Two year's experience in Social Service field preferred. Associate's degree in a related field preferred. Must have excellent computer, communication (verbal & written), clerical and organizational skills. Must be customer service oriented and have excellent people skills. Veteran's status preferred.
