The Nurse Practitioner works in collaboration with the Medical Director and Hospice Team to provide Hospice care services to patients and families in the inpatient setting or in a home setting within the scope of practice of a Nurse Practitioner. Perform procedures for which they are privileged to do and documents assessments, evaluations, consultation reports and progress notes. 

 

A Nurse Practitioner is defined as a registered nurse who performs such services as legally authorized to perform (in the state in which the services are performed) in accordance with State law (or State regulatory mechanism provided by State law) and who meets training, education and experience requirements. 


  • The Nurse Practitioner (NP) performs face-to-face encounters with the patient as a part of the certification process; completes Attestation documentation.
  • When serving as an attending physician, the Nurse Practitioner participates as a member of the IDG that establishes and/or updates the individual’s plan of care.
  • Collects and assesses patient care data, develops and implements a plan of care, manages and evaluates outcome of care.
  • Performs physical and psychosocial assessment on all Hospice patients.
  • Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
  • Assists in developing a pain management and symptom management plan to keep the patient as comfortable as possible. Reassess pain management plan when pain is not manageable on the current medications.
  • Demonstrates the ability to prescribe medications (see individual state laws) and non-pharmacological therapies based on the patient's diagnosis, health history and age.
  • Provides skilled nursing care.
  • Responds to the physical, mental, social and emotional needs of terminally ill patients and their families. Offers ongoing support and palliative care.
  • Follow up with obtaining lab results and notifying physicians of any significant results.
  • Performs patient care responsibilities considering needs specific to the standard of care for patient’s age and clinical condition.
  • Follows the seven (7) medication rights and reduces the potential for medication errors.
  • Provides patients and/or family with health education, counseling and referrals to other healthcare professionals and community resources when appropriate.
  • Participates in case management.
  • Complete patient documentation on time.
  • Completes assigned training on time.
  • Performs other duties as assigned.

  • Must be a graduate of an approved school and licensed as a Nurse Practitioner in the state of Utah.
  • At least 5 years nursing experience with a minimum of one year in Hospice preferred.
  • Minimum of 2 years clinical registered NP experience.
  • Must have demonstrated experience in hospice, palliative care or a related field of nursing such as geriatrics or internal medicine and an understanding and acceptance of hospice principles of care.
  • Must have demonstrated knowledge and well-developed skills in medicine, pharmacology, pain and symptom control, and psychology of loss.
  • Must have knowledge of regulatory requirements and Medicare Conditions of Participation and be compliant with the regulations.
  • Possess an unexpired driver’s license.
