JOB DESCRIPTION
TRANSPORTATION AND OUTREACH COMMUNITY HEALTH WORKER JOB DESCRIPTION –
SUMMARY OF POSITION:
The Transportation and Outreach Community Health Worker (TOCHW) provides assistance and Outreach services to clients and community partners regarding the Healthcare for the Homeless – Houston (HHH) transportation system (Project Access) and HHH medical services. The TOCHW also records and maintains appropriate and assigned data and information related to the successful operation of the system and submits reports promptly as designated. The TOCHW will help increase access to Transportation and medical care for people who have experienced homelessness through outreach, education, and peer support. This position provides a variety of specialized services to special populations. The TOCHW is expected to understand mental illness and addiction and be willing and ready to engage with clients who struggle with these issues at all levels.
RESPONSIBILITIES:
· Travel on the weekly bus route and provide a general source of information and referral to local providers of basic healthcare and homeless services to clients on the bus.
· Outreach to clients and community agencies to promote clinic and transportation services.
· Maintain an appropriate communicative relationship with the Driver of the vehicle providing assistance and support for the successful completion of the daily route
· Assist Bus Driver with supervising client access and exit from the bus at each route stop
· Maintain appropriate and designated reports and data of the number of clients who enroll in the service
· Collect and count “tickets” presented by riders when boarding the Project Access bus, entering daily ticket counts and maintaining data in MS Excel for monthly reports.
· Serve as a liaison between the Bus Driver and HHH to maintain clear levels of understanding about current issues and concerns related to the safe, efficient operation of the system
· Report any problems, whether with passengers or participating agencies, including the bus vendor, to the HHH Director of Social Services.
· Liaison for ticketing sites and all agencies on the bus route in order to maintain a positive and successful transportation system, including but not limited to, monthly form and ticket coordination.
· Outreach to clients to promote bus ridership
· Assist clients in gaining access to and navigating a primary healthcare medical home and other community based social services (i.e., behavioral health services, housing, legal, etc.)
Conduct needs assessments to identify barriers and satisfaction surveys for program evaluation
· Provide culturally and linguistically appropriate services and health education to clients
· Complete accurately, and in a timely manner, all-necessary forms and reports and submits such documentation to his or her supervisor within designated timelines
· Develop relationship with area social service agencies to build knowledge of the resources available to clients
· Participate actively in regular supervisory and team meetings, training sessions, as well as program and operational planning
· Work toward developing a broad knowledge of casework and carries the volume of work as determined with his or her supervisor
· Report any client safety concern to their immediate supervisor without the fear of reprisal
· Support grant/contract management process through alphabetizing of forms, data collection, data entry and reports
· Print, copy, and/or scan forms, flyers, brochures, reports etc., as needed · Answer and screen telephone calls and route calls to the appropriate person · Assist with preparation of materials for meetings, training sessions and workshops when needed
· Maintain a professional disposition while working with a multidisciplinary healthcare team
· Other duties may be assigned
SKILLS:
- Strong organizational skills
- Community Health Worker Certification or ability to obtain in first year of hire.
- Strong interpersonal and social skills with demonstrated ability to collaborate with a variety of individuals from a wide range of professional and personal backgrounds
- Good oral and written communication skills
- Able to operate small office equipment, including photocopier, telephone and personal computer.
- Basic knowledge of Microsoft Office
- Strong project management abilities
- Ability to navigate local transportation services (i.e., bus, train and taxi)
PHYSICAL REQUIREMENTS:
· Lift up to 25 pounds (laptop computer, small office equipment uses, or similar items)
· Must be able to stand, sit, walk, kneel and bend
Ability to move freely on and about a bus
