Job Description Summary / TWC Summary:JOB SUMMARY:The CHW supports the MH-REACHTSU by connecting, building, and sustaining partnerships between individuals with lived maternal health experiences and organizations serving these populations. The CHW performs duties related to the operations, functions, and activities of MH-REACHTSU . This position is supported by a 5-year grant funded by the Health Resources and Services Administration (HRSA). The CHW work will take place in diverse settings including University and community settings.
DISTINGUISHING CHARACTERISTICS/DESCRIPTION:The CHW is a liaison and cultural mediator between the community and academic/research stakeholders. The CHW has a close understanding of the ethnicity, language, socio-economic status, and life experiences of the community served. CHW’s assists with access to needed services through outreach, patient navigation and follow-up, Community Health education and information, informal counseling, social support, and advocacy. CHW’s build systems capacity through program coordination, outreach activities, clinical research recruitment, and training.
This is a Grant Funded position.
- Cultivate and maintain relationships to foster partnerships and recruitment efforts for community advisory boards, community engagement studios, and research participation.
- Identify and develop communications that are culturally appropriate to the community.
- Submit monthly reports of progress in achieving individual project goals and work closely with all key personnel to prepare reports.
- Participate in development of training activities for stakeholders including CHW’s, community engagement studio participants, and pilot awardees.
- Organize and conduct community outreach activities including health education on MH issues.
- Attends scheduled team meetings.
- Assists with the coordination and tracking of CEC activities and adherence to timelines.
- Coordinate activities and dissemination strategies to facilitate community’s understanding of MH REACH data and results.
- Performs other job-related duties as assigned.

Hiring Range:Commensurate with experience.
Education:Bachelor’s degree in health related fields including health administration, health promotion/education, social work-related degree and/or Community Health Worker Certification.
Required Licensing/Certification:Community Health Worker Certification or Community Health Education Specialist preferred but not required.
Knowledge, Skills, and Abilities:Knowledge of:
- Social determinants of health and health disparities related to maternal health.
- Basic office procedures, including meeting agenda development.
- Current and evolving issues in the community.
- Networks of organizations and individuals involved in maternal/or Community Health.
Skill in:
- CHW Core competencies; including but not limited to:
- Planning and setting up presentations, educational/training sessions, workshops, and other activities.
- Manage time and prioritize work tasks.
- Problem-solving and decision-making.
- Organizing community to address issues important to the community.
- Coordinate outreach activities and programs.
- Developing individual goals and workplans.
- Verbal and written communication.
- Able to work in a team.
- Work independently to meet expected deadlines and objectives.
Six (6) to twelve (12) months of related community based or similar experience.
Working/Environmental Conditions:- Routine office environment.
- Sitting or standing in one location much of the time.
- Ability to travel locally, using personal transportation, up to at least once/week.
