Job Description
FUERTE Wraparound is a client centered, family oriented, strength based, and needs driven process to aid families in the Juvenile Justice System reach their goals. The Transitional Specialist is partnered with a Mental Health Clinician and works collaboratively with Wraparound Probation Officers to help youth address mental health needs and meet the terms of Probation.
The Transitional Specialist is responsible for coordinating agencies, providers, community resources, and natural supports to serve the youth and families. The Transitional Specialist provides intensive care coordination and linkages to outside resources and community-based services. The Transitional Specialist also supports and encourages youth and family members in practicing new approaches and skills introduced through individual and family therapy. Services are provided in the home and community settings across Santa Cruz County. This position has flexible work hours and rotating crisis on-call shifts. This is a full time position. Hourly pay differential for bilingual skills awarded upon qualification.
Responsibilities:
- Meet with youth and family in the home at least once per week
- Link youth and family to career/life skills trainings and pro-social activities, and outside mental health resources when necessary
- Coordinate client care and monitor referrals as needed
- Work closely with the FUERTE Wraparound Treatment Team (Probation Officer and Mental Health Clinician)
- Attend and facilitate Child and Family Team meetings
- Maintain appropriate documentation of services
- Some evening and weekend hours required
- Represent the agency in a professional and collaborative manner that fosters positive working relationships with other agencies and agency representatives
Qualifications:
Required:
- Bilingual in English and Spanish: Verbal and Written required
- Associate’s or Bachelor’s Degree or equivalent experience
- Thorough knowledge of community resources and services throughout Santa Cruz County
- Valid California Driver’s License and clean driving record
- Reliable vehicle and insurance
Preferred:
- Experience facilitating groups preferred
- Experience working with youth and families involved in the juvenile justice system
- Bachelor’s degree in psychology or social sciences
Wages & Benefits:
Our total compensation package includes a comprehensive medical and dental insurance plan for full time employees, 403(b) retirement plan, paid holiday, sick time, and 17 days of paid vacation, pro-rated for positions less than 40 hours a week.
