We have medical clinics where we apply the research, to empower the aging population (Second-fifty years / 65+) and give them hope and a future, and ultimately restore their ability to Choose How You Age!
From walkers & wheelchairs to dancing again with no assisted devices just 6-monthts later, to working with people who don't want to go down that path, we specialize in the prevention and treatment of Sarcopenia (and associated conditions). Through education, a personalized-plan, state-of-the-art technology, and specialized medical providers we are here to work with people who want to achieve their meaningful lifestyle goals, and not settle for the status quo and the 'usual aging' experience.
For this onsite Personal Care Coordinator (PCC) Position:
Responsibilities
- Help contribute to a positive team / healing environment
- Support with new Patient / Member onboarding
- Help Maintain / Enhance Patient Member Experience
- Help Maintain / Enhance Patient Member Engagement
- Support Planning / Organizing Monthly Celebrations
- Supports Clinicians with chart creation, organization, and management
- Supports Physician & Community Relations Team with prepping reports & data
- Supports Patient / Member Scheduling and Communications
Requirements

Pay: $17.00 - $20.00 per hour
Expected hours: 20 – 40 per week

- 4 hour shift
- 8 hour shift
- Monday to Friday
