Elite DNA Behavioral Health is seeking to hire a full-time Targeted Case Manager for our BHH program for our Fort Myers office.
The Targeted Case management position is a community-based position requiring flexibility of working with both children and adults diagnosed with a mental illness and their support system. Most clients in this program are adults who require stabilization including mental health services to avoid inpatient hospitalization. As a Targeted Case Manager, collaboration and maintaining positive relationships with providers, outside community resources, and collaterals is essential. This position works in collaboration with a licensed therapist. The Targeted Case Manager coordinates and monitors mental health services, advocates for clients within various systems, links individuals to community resources, and provides culturally competent support to engage and expand the individual's natural support system.
Requirements
- Engaging with clients and their families in a person-centered, nonjudgmental, culturally competent, and strengths-based approach
- Completing strengths-based assessments within the first 30 days of providing services to assist with identifying case management needs
- Ensuring service plans for case management services are completed within 30 days
- Regular phone contact and weekly home visits with clients either in the community or via telehealth
- Documenting all contacts with clients via daily progress notes
- Maintaining an active case load and prioritizing duties to ensure deadlines are met
- Attending weekly individual/group supervision with case management supervisor and any additional trainings as required
- Attends biweekly BHH team meetings
- Other duties as assigned
EDUCATION & EXPERIENCE:
- Bachelor's degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field
- One year of full-time or equivalent experience working with adults and/or children with serious emotional disturbances preferred.
QUALIFICATIONS & REQUIREMENTS:
- Valid Florida driver's license and auto insurance
- Knowledge of DSM-V and available community resources
- 50 hours of content-specific training, from an FCB, Approved Education Provider allocated as follows: (to be completed within 3 months of hire)
- 1. Engagement and Assessment (initial and ongoing): 9 hours
- 2. Service Planning and Development (initial and ongoing): 8 hours
- 3. Coordination, Linkage, and Monitoring: 13 hours
- 4. Documentation: 10 hours
- 5. Professional, Legal and Ethical Responsibilities: 10 hours
- Obtain CBHCM (Certified Behavioral Health Case Manager) provisional certification within 3 months of hire and upgrade to full certification within 1 year.
- Complete daily documentation within 24 hours
- Comply with all Medicaid/AHCA/BCBS standards for providing Targeted Case management services and all Elite DNA Therapy policies and procedures
- Demonstrate excellent writing and organizational skills, strong interpersonal skills, culturally competence, a positive attitude, and proficiency with computer skills
- Ability to work both independently and as part of a collaborative team
- Bi-lingual English/Spanish is a plus
COMPENSATION:
Salary range is dependent on experience, education, Wraparound and CBHCM certification status. CBHCM certification is desired but not required.
