- Provide individual and group counseling sessions to patients struggling with substance abuse
- Conduct assessments and develop treatment plans based on patients' needs and goals
- Monitor and evaluate patients' progress throughout their treatment journey
- Collaborate with a multidisciplinary team to ensure comprehensive care for patients
- Maintain accurate and up-to-date medical records and documentation
- Provide education and support to patients' families regarding addiction and recovery
- Utilize case management skills to coordinate care and resources for patients
- Stay current on the latest research, trends, and best practices in addiction counseling
```Experience```
- Minimum of [X] years of experience in substance abuse counseling or related field
- Experience working with diverse populations, including pediatrics, adults, and seniors
- Familiarity with addiction treatment modalities, such as cognitive-behavioral therapy (CBT) and motivational interviewing (MI)
- Knowledge of medical records management, including documentation requirements and confidentiality regulations (HIPAA)
- Strong understanding of case management principles and practices
- Experience working in an acute care or hospital setting is preferred
- Familiarity with managed care systems and insurance processes

- Excellent interpersonal and communication skills to establish rapport with patients and effectively convey information
- Ability to provide compassionate and non-judgmental care to individuals struggling with addiction
- Strong knowledge of physiology and the effects of substance abuse on the body
- Proficiency in using electronic health record systems for documentation purposes
- Ability to work collaboratively as part of a multidisciplinary team
