Reports to: Ke Ola Pono and Case Management Coordinator
Status: Part time, Hourly
Schedule: Varies based on program need and client availability

JOB SUMMARY:
Under the supervision of the Ke Ola Pono and Case Management Coordinator and in coordination with the KOHOM Program Coordinator/Resident Manager, the SLP (Sober Living Program) Case Manager will monitor parolees in the cottage and engage women who are referred to Ka Ohu Hou O Manoa Supportive Living program. The SLP Case Manager will provide case management services, including referrals for substance use treatment, accessing vocational supports/opportunities, family therapy, healthcare, domestic violence supports, sexual abuse services support, and connecting with and utilizing community resources. The SLP Case Manager will be culturally competent and able to provide a gender responsive service approach to high need female parolees and draw on Hawaiian and other Asian and Pacific Islander cultural values that promote a recovery lifestyle.
QUALIFICATIONS: Bachelor's degree in related field, preferred, and/or a minimum of 1 year work experience or knowledge of addiction/recovery and offender services. Personal vehicle and Valid Hawaii Sate driver license required.
ESSENTIAL FUNCTIONS:
  • Regular attendance is required for the position.
  • Provides case management and supportive connections to residents to meet a range of needs including accessing health insurance/health care, domestic violence support, substance use treatment, life skills, pro-social activities, cognitive/developmental resources, sexual abuse services, family therapy, and vocational counseling.
  • Provides in-facility connection with resident prior to residents' release from incarceration.
  • Provides orientation and supervision of SLP residents at KOHOM for adherence to SLP rules/structure.
  • Maintains open communication and coordination with parole officers and other treatment providers on resident's progress. Provide reports as required through HPA contract and SAFTS policies.
  • Utilizes motivational interviewing techniques to support residents in moving toward reintegration and self-sufficiency through the development of recovery plans with goals/objectives and is coordinated with supervision parole officers' input and support.
  • Models and coaches' appropriate communication, problem solving and conflict resolution skills for residents while upholding program rules and expectations.
  • Participates in weekly meetings with the KOHOM team that includes the resident manager, program coordinator and clinical director to discuss cases as well as give feedback regarding facility inspections and programmatic policies.
  • Tracks data, client services, and maintains appropriate client records on WITS and meets all other SAFTS documentation and reporting requirements (such as quarterly record reviews and audits)
  • Participates in clinical treatment team meetings, as appropriate.
  • Maintains work area in a neat, orderly and sanitary manner.
  • Correctly and promptly follows specified agency procedures required for the appropriate handling of all Emergency Response Codes.
  • Performs other related duties as may be required or assigned.

OTHER REQUIREMENTS:
Able to meet basic standards of physical, mental health, and communication demands as specified by State and/or professional accreditation standards applicable to this agency. Initial/Annual criminal background check, pre-employment, annual physical examination, CPR/first aid, TB test, and driver abstract compliance with agency policy are required.
