Waikiki Health is recruiting a part-time or full-time HIV Medical Case Manager

The pay rate for this role is $22.50 - $24.50 per hour.

Schedule: 20-40 hour work week

The HIV Medical Case Manager will work with members of the interdisciplinary team to provide quality care to patients living with HIV/AIDS and affected others to achieve optimal clinical outcomes. The HIV Medical Case Manager will provide medication adherence counseling, health education and care management; coordinate the implementation of individual care plans and monitor referrals. The HIV Medical Case Manager reports directly to the Ryan White Program Director.

Primary duties include but are not limited to:

  • Engaging in a multi-disciplinary team approach in the development and management of care plans and appropriate medical and psychological interventions and referrals for people living with HIV/AIDS.
  • Coordinate and collaborate internally and externally with members of patient's care team (medical providers, BH providers, enabling staff, Case Managers, caregivers, and other authorized individuals) to address individual needs to improve retention in care, health outcomes, and quality of life.
  • Assess eligibility for the Ryan White (RW) program services.
  • Assesses patient adherence to HIV medication and education.
  • Document encounters appropriately in EMR and perform coding activities for RW eligible services, reviews clinical records and performs coding activities monthly.
  • Educates patients, provides community resources (counseling and referrals), and participates in testing services for various diseases.
  • Participate and assist in coordination of team meetings, support groups, community events, staff training, and development opportunities.
  • Participate in routine quality improvement activities and data collection, review, and reporting for federal and state grant requirements.
  • Provide feedback and participate in the creation of written materials.

Qualifications:

  • Bachelor’s degree in social work, Public Health, or other relevant concentrations from an accredited university; 2 years of relevant social work and/or health care experience may be substituted.
  • Case management experience.
  • Strong computer skills in MS Office - Excel, Word, Outlook, and the clinical system: Athena (helpful)
  • Effective (clear and concise) written and verbal communication skills.
  • Valid driver’s license and a clean DMV record – minor violations allowed. This position may be required to work in the field, a company car is available.

Preferred and helpful qualifications

  • Experience working with individuals with HIV/AIDS.
  • Knowledge of basic medical terminology.
  • Flexible hours, including some weekend availability.
