Position:Nurse Case Manager
Location: Honolulu, HI
Pay: $65,000/yr
Experience: One (1) year of RN experience. One (1) year of volunteer or work experience.
Education: Bachelor’s degree in the field of nursing from an accredited college or university; current. Hawaii state license as a registered nurse.
Type: Full-time; Direct Hire
Schedule: Monday to Friday, Flexible Schedule
Kahu Malama Nurses is seeking a Nurse Case Managerto join our growing and dynamic team!
Job Description:
- Develop, implement, evaluate, and update resident's Health Maintenance Plan.
- Ensure that residents are protected from abuse, neglect, and hazardous conditions.
- Maintain documentation in resident medical records for all individuals.
- Ensure medication orders are filled accurately and received on time.
- Observe and assess residents' condition through annual exams and provide direct care whenever necessary.
- Review resident health, safety, medication administration, and infection control documents.
- Delegate nursing tasks to personnel through proper training, documentation, and performance assessments.
- Assess the effectiveness of medications, address concerns, and communicate with physicians and pharmacists.
- Ensure program and facility compliance with all applicable standards, policies, and regulations.
- Other duties as assigned.
Position Requirements:
- Possess a smartphone (required).
- State of Hawaii driver’s license and Visions certification.
- Current work clearances (PE, initial 2-step PPD, CPR, First Aid, etc.).
- Proficiency in computer software programs related to job duties.
- Ability to operate standard office machines, household appliances, and assistive devices (Hoyer lift, wheelchair, walker, and gait belt).
- Strong verbal, written, and interpersonal skills.
- Ability to organize, prioritize work, and meet deadlines.
