The Day Habilitation (Day Hab) Supervisor will report to the Director of Community Programs. The Day Hab Supervisor will be responsible for the daily operations of the program without walls setting ensuring the implementation of all volunteer & recreational activities. The Day Hab Supervisor will also make connections in the community, acquire additional volunteer activities and take part in recruitment of participants and staff. The Day Hab Supervisor will also provide direct support including assistance with ADL's to all participants when necessary.
RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:- Ensure the Implementation of activities based on program's scheduled activities.
- Provide direct support to participants in their daily activities when necessary.
- Develop relationships and volunteer opportunities for participants in the community.
- Ensure protective oversight for all participants as per their LIfe Plan
- Drive agency vans and demonstrate safe driving practices. Ensure cleanliness of vehicles.
- Review completed day hab documentation and van logs daily per agency and OPWDD guidelines.
- Ensure completed day hab documentation and van logs are submitted on a bi-weekly basis per agency and OPWDD guidelines,
- Provide ongoing communication with Director of Community Programsregarding participants and activities.
- Attend scheduled trainings and staff meetings.
- Report any issues with program, vans, and participants to Supervisor immediately.
- Maintain compliance of day hab setting and documentation
- Actively and continuously identify and connect with volunteer opportunities in the community
- Oversee the overall daily operations of the day hab program.
- Perform other dutiesas assigned & necessary
- Salary starting at $20 per hour

- Associate's Degree or Higher plus 2 years of supervisory experience orHigh School Diploma or equivalent with a minimum of 5 years supervisory experience;OPWDD settings preferred
- Prior experience assisting persons with IDD/Autism preferred
- Excellent communication and written skills
- Knowledge of Microsoft office, Website navigation and other software technologies
- Able to provide ADL and Direct Care assistance when needed
- Valid NYS Driver's License
