Form of workBurkwood Treatment Center is seeking a WI licensed Substance Abuse Counselor (SAC-IT, SAC, or CSAC) to provide treatment and support to assist patients in their recovery from addictions, such as alcoholism and drug addiction.

ESSENTIAL JOB FUNCTIONS:
- Conduct individual and group counseling sessions in a residential treatment setting.
- Responsible for initial assessments, as well as follow-up assessments for patients.
- Prepare individual treatment plan for each patient.
- Maintain patient evaluations, notes, and reports as required by the facility and the State of WI.
- Provide crisis intervention to patients, as needed.
- Provide case management duties for patient, ensuring individualized quality care.
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
- High school diploma or equivalent required.
- Degree in social or health services field from an accredited college or university required.
- License in the State of Wisconsin as a SAC-IT, SAC, or CSAS required.
