Position: MDS Coordinator

Pay: Starting at $40/hr
Experience: Ten (10) years of experience as an RN. Previous management experience
Education: Bachelor’s degree in nursing. Current state licensure as a registered nurse (RN)
Type: Full-time; Direct Hire
Schedule: Monday – Friday, 8:30am to 4:30pm
Kahu Malama Nurses is seeking an MDS Coordinator to join a growing and dynamic team!
Job Description:
- Assess and analyze resident data
- Ensure that residents receive the highest quality of care tailored to their specific needs
- Work closely with the interdisciplinary team to develop individualized care plans
- Monitor the effectiveness of the care provided
- Ensure accurate and compliant documentation of resident assessments and care plans
- Other duties as assigned
Position Requirements:
- Current state licensure as a registered nurse (RN)
- Strong organizational and communication skills
- Excellent attention to detail and accuracy in documentation
