Position: Registered School Nurse
Location: Wailuku, HI
Pay: $31.73/hr
Experience: Two (2) or more years of experience as a Registered Nurse (preferred). Experience or training in case management (preferred).
Education: Graduate of an accredited school of Licensed Professional Nursing. Licensed in the State of Hawaii as a Registered Nurse.
Type: Full-time; Direct Hire
Schedule: Monday – Friday, 7am to 3:30pm
Kahu Malama Nurses is seeking a Registered School Nurse to join a growing and dynamic team!
Job Description:
- Provide patient care which includes setting patient expectations, providing education, gathering data, and care coordination.
- Coordinate pre-visit planning with the medical assistant and medical provider.
- Utilize the nursing process of assessment, planning, intervention, and evaluation.
- Plan and organize small and large-scope projects.
- Work closely with an interdisciplinary team including the medical care teams, Integrated Health team, Dental staff, Member Services, and outside referral entities.
- Handle duties in a team-based care model by functioning as the primary care facilitator.
- Prepare for, attend, and participate in team meetings and huddles.
- Other duties as assigned.
Position Requirements:
- Understand the fiscal and programmatic administration of a federally funded Community Health Center.
- Computer literate in word processing and spreadsheets.
- Possess strong leadership skills and ability to solve problems.
- Excellent verbal and written communication skills.
- Ability to make accurate assessments and prioritize work to be done.
- Excellent interpersonal and communication skills.
- Ability to work independently and with a team.
