Job Summary:
Here at Hannah Home Health Care, it is our mission to provide our patients with the utmost in quality professional care. As a Start of Care Coordinator, you will have the opportunity to collaborate directly with our clinical leadership team and Chief Clinical Officer (CCO), towards the accomplishment of this goal from the very beginning of the admission process. Our passionate team members will depend on you to ensure that the agency obtains all needed documentation including orders, H&P, medication lists and any other necessary information required for admission, as well as reliable problem solving skills to ensure a smooth and efficient start to care.
On an average day, you will:
- Apply specific criteria for the admission to home health care, utilizing Chapter 7 guidance to establish appropriate levels of Care and patient's eligibility for services.
- Establish and maintain positive working relationships with current and potential referral sources.
- Gather and input all proper documentation from referral sources required for admission of patients.
- Assign referrals for each location in our service area, communicating schedules to Start of Care RN's in a timely manner for admission to home health care.
- Inform and confer with the Clinical Leadership Team and CCO on all patient, personnel, and service management needs and problems.
- Assure confidentiality of all patients, physicians, staff and proprietary information in line with HIPAA regulations.
- Gather, collate and report referral statistics including key customer referral trends.
Requirements:
- Associates Degree of equivalent years of training and work experience.
- Minimum of 1 year experience in healthcare or related industry
- Demonstrate an understanding of state and federal home care standards, regulations and Chapter 7.
- Excellent oral and written communication skills with clinical and non-clinical staff
- Strong organizational skills, attention to detail and computer software proficiency

Pay: $20.00 - $22.00 per hour
Expected hours: 40 per week
