Speech Therapist - Sourcing

Freedom Therapy is now hiring PRN Speech Therapiststo join our growing team in the Rocky River, Parma, Mentor, and Beachwood, Ohio areas!

In this role, you will be part of an interdisciplinary team that focuses on providing compassionate quality care and producing positive outcomes for your patient population. Home care provides context and real-world perspective about what will really help patients restore their health.

The right candidate will have:

• Must possess a bachelor's or master's degree in speech therapy approved by an accredited organization
• One year experience as a Speech Therapist in an acute care setting preferred
• Community/home health experience preferred
• Must possess current license as a Speech Therapist from the State of Ohio
• Must possess valid driver's license
• Must have the use of an automobile in good working condition and automobile insurance
• Satisfactory criminal background check
• Maintains current CPR certification

Our Speech Therapist will:

• Review patients' medical histories
• Diagnose patients by observing their movements and listening to their concerns
• Develop individualized treatment plans for patients
• Outline clear goals for patients and the expected outcomes of the plan
• Provide speech therapy services to patients according to a Plan of Care
• Initiate speech therapy program and instruct other personnel, patient and/or family/ caregivers in therapy program, as well as the goals of therapy
• Confer with the attending physician, home care team members and patient and/or family/caregivers to promote coordinated and comprehensive care
• Prepare and submit clinical and progress notes in a timely manner and in accordance with Freedom Therapy, state and federal guidelines and regulations
• Instruct patient and/or family/caregiver and other team members in the patient's treatment regimen as indicated
