Social Worker/Discharge Planner

Foss Home and Village a non-profit memory care, long-term care and assisted living facility serving our community since 1929, is seeking a full-time Social Worker/Discharge Planner. This is an excellent opportunity to work with residents.

This position requires a Bachelor's Degree in Social Work or in the Human Services field, a minimum of 1 year experience in a long-term care facility, strong communication, organization and customer service skills, knowledge of Medicare, Medicaid and Home Health

SUMMARY OF JOB:

Responsible for planning and coordinating patient discharges from short term rehabilitation in a manner that supports patients in maintaining and enjoying the highest possible level of physical and psychosocial well being.

KEY DUTIES:

• Serve as advocate for each resident.
• Provide social services to include individual and family support and problem solving; discharge planning; crisis intervention for resident and families.
• Completes all necessary paperwork for new admissions and coordinates with other departments as needed.
• Facilitates transfers and discharges for all short-term rehabilitation residents, to include arranging home health services and ordering necessary equipment.
• Complete psychosocial assessments on admit and annually thereafter. Complete quarterly social service care plan and additional progress notes as needed.
• Liaison with mental health consultants.
• Participates and/or facilitates resident’s interdisciplinary plan of care meetings. Attends other meetings as assigned.
• Perform other relevant social services or other duties as assigned by management to meet changing job-related requirements.

COMMUNICATION: Responsible to maintain communication of a frequent and significant nature, requiring thorough knowledge of facility policy.

EDUCATION: A bachelor’s degree in social work or a bachelor’s degree in human services field including, but not limited to, sociology, special education, rehabilitation counseling and psychology.

EXPERIENCE: 1 year.
