KVH is an integrated healthcare delivery system that includes 24-hour emergency care, inpatient, and outpatient hospital services through KVH Hospital. Job DescriptionAssess patients over the age of 65 and those who have been referred to the Social Services department. Develop discharge plans for those patients requiring additional services or intervention related to their chosen disposition. Work with families and staff to offer information on community resources and/or grief support. Respond to and work with CPS/APS referrals and patients involved with CPS/APS.
QualificationsWhat is required...
- Bachelor of Science or Art (preferably in Social Work)
- Previous hospital or nursing home experience
What is preferred...
- Master of Social Work degree from an accredited school
- Minimum of two years’ experience in human services field
- Licensed Independent Clinical Social Worker (LICSW)
Additional Information
Wage: Wage range depends on licensure and will be based on years' of experience.
Social Worker starts at $28.51/hour and goes up to $43.19/hour.
Social Worker (MSW) starts at $30.89/hour and goes up to $46.82/hour
Social Worker (LICSW) starts at $33.89/hour and goes up to $51.36/hour
FTE: 1.0 = 40 hours/week
