Summary: Provide discharge planning and biopsychosocial services to NICU patients/families. Provide information and guidance to patients, families, and other hospital departments regarding all aspects of patients' post-discharge plan including but not limited to therapies, adoption, commonly required home care modalities, continuous oxygen therapy, electronic cardio-respiratory monitoring, technologically assisted feeding and intravenous therapy, surrogacy, assessments, durable medical equipment (DME), support groups and services, placement, home health, follow up appointments, and referrals.
Qualifications: Minimum of 6 months experience working in Level III/NICU as a Social Worker/discharge planner. Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW). Masters of Social Work (MSW). Current CPR.
