The Social Worker MSW will work 2 hours per week supporting DaVita SNF (Skilled Nursing Facility) dialysis dens. This person will visit the dialysis den on-site once per month. The DaVita dialysis dens are located at Clear Creek in Westminster, CO & Harmony Pointe in Lakewood, CO.

Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing a significant burden on patients and adding significant avoidable costs to the healthcare system. Requirements:

• Master's degree in Social Work (MSW) required with a specialization in clinical practice.
• Meets all state-required licensure regulations to practice in the dialysis setting.
• Demonstrated knowledge of government and private insurance programs
• Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days

• Compensation for the role will depend on a number of factors, including a candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown.
