We are seeking a Social Worker to join our Hospice team! You will provide emotional and administrative support to clients in either individual or group settings. This position is paid per visit. We currently have an opening for Full Time, Part Time, or PRN.
The Medical Social Worker is responsible for the implementation of standards of care for medical social work services.

Responsibilities:
- Assess the nature of patient's situation related to the patient's illness and environment by interviewing and reviewing personal history and communicate findings to the Registered Nurse.
- Carry out social evaluations and plans intervention based on evaluation findings.
- Maintains clinical records on all patients referred to social work.
- Prepares clinical and progress notes.
- Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs.
- Provides information to patients or families/caregivers and community agencies.
- Serves as liaison between patients or families/caregivers and community agencies.
- Maintains and develops contracts with public and private agencies as resources for patient and organization personnel.
- Maintains collaborative relationships with organization personnel to support patient care.
- Participates in the development of the total plan of care and case conferences as required.
- Assists physician another team members in understanding significant social and emotional factors related to health problems.
- Participates in discharge planning.
- Other duties as delegated by the Clinical Director/Supervisor.
- Develop and execute individual treatment plans.
- Determine appropriate milestones to gauge client progress.

Qualifications:
- Bachelor's degree in social work from a school of social work accredited by the Council on Social Work Education.
- Minimum of one year experience in healthcare
- Experience in Home Health care is preferred.
- Previous experience in social work, counseling, or other related fields is a plus.
- Demonstrates good verbal and written communication, and organization skills.
- Possesses and maintains CPR certification.
- A current MSW license in the state of Colorado
- Transportation that is in working and insured
- Compassionate
- Ability to build rapport with clients.
Compensation:
- $61-$105, depending on experience!
